Asymptomatic polypoidal intracavitary cardiac metastases from pulmonary adenocarcinoma

A young male presented with right flank pain, fever, cough, loss of weight and appetite. CT of the abdomen revealed a right suprarenal cystic mass measuring 10×8 cm with para-aortic lymphadenopathy along with inferior vena cava (IVC) thrombus. The patient died following cardiac arrest.
At autopsy, the right and left adrenals weighed 160 g and 11 g, respectively. The right adrenal mass (9×8 cm) had a variegated appearance with geographic areas of necrosis. Histology revealed peripherally compressed adrenal cortex with tumour cells showing marked atypia. A thrombus was seen in the right adrenal vein, extending to the IVC.
